In answer to the question on that page:
Question A: "What should be the right structure of the scenario?"
In my experience we have normally gone with the second approach, but if 
you have thoughts that would be better formatted in the first approach, 
then feel free to do that. Go with whichever you think communicates the 
scenario most clearly, as clarity is very important when discussing these 
scenarios.

Question B: "What should be the ascertainment of the scenario?"
I would suggest a "concrete" scenario to start with (i.e. along the lines 
of "TWS requests that the ABC workflow is started"), and then (if it helps 
clarify the scenario) you can follow up with an "abstract" scenario (e.g. 
"A consumer requests that the chosen workflow is started"). 
In either case I'd suggest trying to avoid prescribing the precise 
technical solution too early (e.g. whether the type of action is a 
parameter or a separate Plan), but only if it's simple to do so (don't 
make the scenario needlessly complex just to avoid this).
